DAS Health Partners with LinkedMD to Provide Chronic Care Management Services


DAS Health, a leading provider of health IT and management solutions nationwide, has added Chronic Care Management (CCM) to their service offerings in partnership with Tampa-based LinkedMD. DAS Health will offer LinkedMD’s comprehensive CCM program to their clients including physician practices, medical groups, hospitals and Accountable Care Organizations.

LinkedMD provides CCM tailored to those services encouraged by Medicare through CPT Code 99490. As a LinkedMD value added reseller, DAS Health will educate physicians and their practices on the benefits of chronic care management as well as implement LinkedMD’s CCM program.
