Resident's death at Wisconsin memory care facility raises security questions

Patient froze to death in December after wandering outside the facility


A Sun Prairie, Wis., assisted living facility was cited by the Wisconsin Department of Health Services (DHS) when a dementia patient froze to death in December after wandering outside the facility, according to an article on the NBC15 website.

According to documents from the DHS, Faith Gardens had documented the 84-year-old woman moving throughout the facility on several occasions, and knew she had a tendency to wander.

On the night she escaped, it appears the front doors were propped open and alarms weren't activated, according to the DHS.

The Notice of Violation will not allow the facility to take in any new patients and requires Faith Gardens to pay $4,200.
