• An outmigration of care that is bringing healthcare services back to where people live and work

•  Mobile devices, tablets, video streaming and electronic medical records offer alternatives to traditional clinic visits

• The conversion of former big-box retail spaces and decentralized care models that deliver services closer to the home

• A medical home model in which a primary care provider coordinates care with specialists, nurses and other professionals
