Q: Who can do the annual and monthly checks on a generator in an ambulatory surgery center?

A: Well… those are not checks, but actual load tests. According to NFPA 110-2010, 8.4.8 the routine maintenance and operational testing program shall be overseen by a properly instructed individual. Your AHJ has the right to require you to provide documentation that the individual you choose to conduct the monthly load tests has been properly instructed to do so.
