As a result of a recent data security incident, an unauthorized party accessed Community First Medical Center’s (CFMC) computer systems. Upon detecting the unauthorized activity, CFMC immediately contained the incident and commenced an immediate and thorough investigation. As part of their investigation, they engaged leading cybersecurity experts to identify what personal information, if any, was involved.
After an extensive forensic investigation, CFMC discovered on or about July 28, 2023, that one or more of the files potentially accessed by the unauthorized party on or about July 12, 2023, contained personal information pertaining to certain individuals, such as full names, telephone numbers, email addresses, Social Security numbers, medical record numbers and Medicare numbers. The elements of personal information involved varied per individual.
CFMC has no evidence that any personal information has been or will be misused as a direct result of this incident. However, out of abundance of caution, commencing on September 26, 2023, CFMC notified individuals whose information may have been included in the files potentially accessed by the unauthorized party. Notified individuals have been provided with best practices to protect their information, and individuals whose Social Security numbers were contained in the impacted files have been offered complimentary credit monitoring.
