Things to know about the new scores include:
• For the first time, patients to see a hospital's current score alongside its previous scores.
• For the fourth time in a row, no hospitals in Washington, D.C., earned an A grade.
• Hospitals have not significantly improvements on preventing errors, accidents and infections.
