OSF HealthCare to Rename OSF Transitional Care Hospital in Illinois

The facility is being renamed to align better with the other hospital names throughout the rest of the OSF Ministry.

OSF HealthCare is changing the name of its long-term rehabilitation hospital in Peoria to OSF HealthCare Divine Mercy Continuing Care Hospital. 

The move was made to better align with hospital names throughout the rest of the OSF Ministry. The name change will not impact patient care. 

Formerly OSF Transitional Care Hospital, the facility was fully integrated into the OSF Ministry in 2022.  

OSF HealthCare Divine Mercy Continuing Care Hospital provides care for patients who need extra support as they transition from one care setting to the next. The 47-bed hospital offers two levels of specialized care including an 18 long-term post-acute care beds that offer the same intensive care a person would receive in a traditional hospital intensive care unit, but for an extended recovery period; and a 29-bed acute rehabilitation unit for people who have experienced the effects of an acute injury, impairment or illness.
