Two reports found the department’s police ranks suffered from staffing shortages, millions in wasted overtime, inspection failures and oversight caused by a confusing command structure.
Individual executives in the Veterans Health Administration (VHA) set different standards for hospital policing.
One report also found the VA failed to develop adequate threat assessments and written policies, contributing to security vulnerabilities.
