WelbeHealth Opens Two New Centers in California

The two new centers are located in Elk Grove and San Bernardino.

WelbeHealth announced the opening of two new centers in Elk Grove (Sacramento County) and San Bernardino (San Bernardino County).   

Elk Grove, the second-largest city in Sacramento County, has grown to nearly 183,000 residents, with about 24 percent aged 55 and older. In San Bernardino, east of Los Angeles, rates of chronic illness are among the highest in the state. 

Together, these new centers will bring WelbeHealth's integrated Program of All-Inclusive Care for the Elderly (PACE) model, coordinating medical, social and in-home support under one roof. 

PACE is a model of care that addresses the gaps left by the underfunded and fragmented long-term care system across California and the United States.
