West Health Joins Sharp Memorial Hospital to Create Integrated Geriatric Care

New integrated care model to provide comprehensive approach to geriatric care.

By HFT Staff

West Health and Sharp Memorial Hospital will work together to create a new integrated geriatric care model spanning the entire health care continuum.

With support from West Health, all hospital emergency rooms in San Diego—including Sharp’s Emergency Departments—have already achieved Geriatric Emergency Department accreditation by the American College of Emergency Physicians and have committed to providing emergency care that takes into consideration the unique needs of older adults.

West Health is also involved with the development of California’s Master Plan for Aging. By joining with organizations such as Sharp, West Health can apply funding and expertise on projects that have meaningful and long-lasting impact on patient care.
