The Overton Brooks VA Medical Center in Shreveport, La., recently failed a healthcare inspection by the Office of Inspector General, with findings of "a lack of proper cleaning," according to an article on the Shreveport Times website.
The inspection focused on the hospital's compounding pharmacy.
The findings included:
• a lack of proper cleaning of the compounding rooms
• incomplete air and surface testing and certification in the compounding areas
• continued noncompliance with recording cleaning logs since September 2016
• four days where "there was no evidence of appropriate cleaning and mopping of floors"
