The Pennsylvania Department of Health's new report said Geisinger Medical Center had unsanitary equipment and dirty linens on the floors, according to an article on the Citizens Voice website.
Health officials also cited the failure of hospital officials to follow state law for reporting disease outbreaks.
The report was released after a bacterial outbreak that killed three premature babies and sickened five others at the Danville hospital.
The new report says the hospital’s pharmacy did not have a registered professional nurse or dietitian in charge of formula preparation and “failed to clean and sanitize the equipment” used to prepare donated breast milk and infant formula.
