Next Level Medical, LLC (NLM) is providing notice of a recent data event that may affect the security of information related to certain individuals. It is providing information about the event, its response and steps potentially affected individuals may take to help protect their information.  

On July 11, 2026, and then again on July 29, 2026, NLM became aware of suspicious activity within our environment. NLM promptly launched an investigation with the assistance of third-party cybersecurity specialists who brought NLM’s systems back online in a safe and secure manner. On August 31, 2026, NLM determined an unauthorized actor copied certain files from its environment. To date, NLM has no evidence of identity theft or fraud related to this event.  

NLM is undertaking an extensive review of the affected files to determine what information was present and to whom it relates. Once completed, it will work to confirm the identities of, and appropriate contact information for, those potentially impacted and provide notice via written letter where contact information is available. I 

NLM’s investigation and review are ongoing. While the data involved may vary by individual, it may include, but is not limited to, name, Social Security number, date of birth, medical information, health insurance information and other demographic information.
